Annual Vehicle Inspection Report 3-ply Carbonless.
Effective 9/5/12, the U. S. DOT's annual inspection standards for service brakes will change.
Nothing on the front side of J. J. Keller's Annual Vehicle Inspection Report is affected, and this form remains in full compliance with federal requirements.
A portion of the brake inspection information appearing on the back of this form (in particular section 1.a(5) of Appendix G) is what is changing. Due to the short notice of this change, the Annual Vehicle Inspection Report was printed before the change occurred.
To ensure you have the latest information, click the "Updated Information" link above.
Helps you meet DOT's annual vehicle inspection requirements under s396.21. Also includes Minimum Periodic Inspection Standards (Part 396, Appendix G to Subchapter B) on the back of the forms to reinforce the vehicle inspection requirements. The durable 100 lb. manila ply helps preserve the report.

Hey customer care, man you guys are the bomb. I had no idea it would be so straight forward. fantastic! Thanks so much for your help. I'll defiantly keep you all in mind if I need anything. Perhaps I'll go to the site and see if you have a rocker switch that lights. I've put a 12V lighter receptacle and a dual USB port in my creation and wired in a couple of small 12V lights ( from my ho scale model railroad layout) to indicate power on/off. My s/o says they look cool so a lighted rocker would complete the effect eh! Again thanks.
Sincerely, Roger K Roger K
